Radiology Second Read of Outside Images for Trauma

Purpose

  • To reduce the number of repeat imaging performed on patients transferred to the Ryder Trauma Center from outside institutions
  • To replace the need for a second study being obtained in patients who have already undergone appropriate imaging at an outside institution
    • Note: If the trauma team plans on ordering a follow up study, they should not request a second read. If a follow up study is ordered, the second read may be cancelled by the Radiology department.

Procedure

  • Image Uploads (Note: this applies to any outside images on a disc)
    • The trauma team will deliver the disc to the ER Radiology desk.
    • ER Radiology desk can upload images to PACS around the clock.
  • Second Reads
    • Second reads by Radiology will only be done for CT and MRI studies.
    • For any patient requiring urgent evaluation (i.e., patients in TRU, TICU, or any patient with an outside pan scan):
      • Trauma team will enter the appropriate order below in Cerner:
        • CT Second read Emergency/Trauma
        • MRI Second read Emergency/Trauma
      • Imaging study will be placed into the ER Radiology queue.
    • For non-urgent CT and MRI second reads:
      • Trauma team will enter the appropriate non-urgent order (by subspecialty) below in Cerner:
        • CT Second read Body
        • CT Second read Cardiothoracic
        • CT Second read Musculoskeletal
        • CT Second read Neuroradiology
        • MRI Second read Body
        • MRI Second read Cardiothoracic
        • MRI Second read Musculoskeletal
        • MRI Second read Neuroradiology
      • Imaging study will be placed into the inpatient radiology queue.
  • Outside Imaging Reports
    • If the report from the transferring institution is available, this should be taken with the disc to the ER Radiology desk for upload with the images.
    • Every effort should be made to obtain a copy of the outside report and submit to Radiology for upload.
    • The second read will may be addended or held in preliminary status until the prior report is available.
  • For questions regarding an urgent second read, contact the ED Radiologist on call.
