Venous Thromboembolism (VTE) Screening Guideline

Purpose

  • Identify patients at high risk for VTE and intervene early.

Procedure

  • Use dot phrase “.ticuscreenV1” – can be copied on Cerner from Brandon Masi Parker under the “autotext copy utility” function
  • Patients should be screened on admission and weekly (On TUESDAYS)
  • For Trauma patients:
    RAP Score (Risk Assessment Profile for Venous Thromboembolism)
    -Calculate using: https://www.medicalalgorithms.com/risk-assessment-profile-rap-of-greenfield-et-al-for-venous-thromboembolism-in-adult-trauma-patients
    -Refer to AIS Scoring handout below for AIS
    RAP Score >10 denotes high risk. In these patients DVT screening is recommended.
  • Order: US Duplex Veins Lower Ext Bilateral
    • Indication- “Rule out DVT- Screening”
  • Order: Urine Creatinine 24hr
    Order: Vitamin D25 Hydroxy level
  • For Non-trauma patients:
    Caprini Score
    -Calculate using: https://www.mdcalc.com/caprini-score-venous-thromboembolism-2005
    -Patient score: _
    Caprini score >9 denotes high risk. In these patients DVT screening is recommended.
  • Order: US Duplex Veins Lower Ext Bilateral
  • Order: Urine creatinine 24hr
  • Order: Vitamin D 25 Hydroxy level
